2019-2020 Athletic Packet
2020 Player Contract
Must have paperwork before the lacrosse season can begin.
1) A up to date Athletic Packet 2020-2021 completed and at school on file.
2) Physical Examination Form - Completed and at school on file.
3) Emergency Contact Information form - completed ( ALL lines completed ) - This is very important - the information on the forms is used, if an emergency should arise, to contact parents, while the lacrosse team is way from GHS and to help with medical information if the need should arise.! No form, no travel!
